PARENTAL AGREEMENT & POLICIES

 

PARENTS AGREEMENT: I understand that I as a legal guardian of my child hereby expressly acknowledge that the activities involve risks, and I do hereby voluntary assume any and all risks, such as injury, death, disability, to my child and or my child's property, which may occur from participation from these events. My personal insurance bears primary responsability in case of an accident. I release SPORTS FOR KIDS, its volunteers, agents, officers, directors, partners, coaches and assistant coaches of all liabilities arising from this program.

 

 

PROGRAM'S POLICY
Uniforms  All players must wear a complete uniform for practices and games, if the player does not have the uniform he/she will be warned.

Refunds    Sorry! no refunds or credits will be issued for any reason.
Returned cheks    There will be a $30.00 charge for all returned checks.
Enrollment for Park Program   Acceptance of enrollment & payment does not guarantee the same coach your child had previously, game location, or game time and day.
Make-up Practice & Cancellation for Park Program     There will not be any make-ups by Sports For Kids due to an inclement weather reason. If there are any other reasons for cancelation by Sports For Kids then there will be a make-up at a later date.

Make-up Practice & Cancelations for School Program   Cancelations will be made by the school due to an inclement weather. If there is any cancelation by Sports For Kids for any other reason there will be a make-up date at the end of the season.

 

 

By reading and accepting The Sports For Kids Program Parental Agreement & Policies in the registration form you agree with all of the above.
